There are occasions where newspaper, radio or television reporters may visit schools to report on school programs or Friends of King District initiatives. The FOKS may also wish to use photographs, video or audio recordings of students in educational or promotional materials.

Parent/Guardian Acknowledgment and Permission

As a parent or legal guardian of the above student, I understand that Friends of King/Dr. Martin Luther King Jr. School (Elementary and High) has implemented technology protection measures to prevent students from accessing materials on the Internet that may be objectionable, but that such measures may not be one hundred percent effective at all times. I understand that these Acceptable Use Policy restrictions and guidelines are, therefore, a necessary component in protecting my child's activities. I also understand that students and parents/guardians may be held liable for violations of this policy. Finally, I will help provide guidance for my child regarding the use of the Internet in order to further protect and educate him/her regarding risks, appropriate behavior, and legal responsibilities.

I have ready and agree to comply with permissions and restrictions of the Friends of King /Dr. Martin Luther King Jr. School (Elementary and High)

If the student is under the age of 18, please indicate whether or not you grant permission for the student to independently operate computers/technology and to access the Internet at school.
